Northwoods Volunteer Service Award Nomination

In 2003, the President’s Volunteer Service Awards were established to recognize the valuable contributions volunteers are making to our nation. These awards are designed to thank and honor Americans who engage in volunteer service.



As a certifying agency for the President’s Volunteer Service Award, Marshfield Clinic Center for Community Outreach would like to assist Northwoods Coalition members in recognizing exceptional coalition volunteers. If your coalition has an individual who is a long time volunteer, please consider nominating them. The Presidents Call to Service Award, also referred to as the Lifetime Achievement Award, is awarded to individuals who have completed 4,000 or more hours of service over their lifetime.



Please complete and submit the attached nomination/verification form no later than May 28, 2010. Following that date, nominations will be processed and recipients will be notified and invited to attend a recognition ceremony held during the Wisconsin Prevention Conference in Wisconsin Dells on June 22-24, 2010.
